 Copy of your passport or cedula and a copy of your child's passport or cedula. |
| Proof of your child's enrollment, days of attendance/tardiness, and grade level. |
| Your child's most recent progress reports, report cards and/or teacher observations. |

Miscellaneous entries where applicable (academic, behavioral, and/or psychological evaluations)

| Once your child is accepted, we ask that the following documents are sent to us for your child's file: |
| Up-to-date immunization records. |
| Proof of physical exam (which includes but is not limited to a hearing and sight exam) |
| Permanent record information (school cum. file, any disciplinary action reports, any school incident/accident reports, teachers' observations/notes, updated progress report,/report card, etc.) |
Recent portfolio work (samples of your child's latest and favorite class work, projects, written pieces, etc.)
